For your convenience, we have attached our Informed Consent, Manual Lymphatic Drainage, Adult Client Intake, and Minor Client Intake forms (for clients under 18 years of age). If you are able, print out and complete the Informed Consent and appropriate intake form and bring them with your to your initial massage session. Please note that filling out your health history form to completion will insure the most personally-tailored massage session possible.
